We are delighted that you will be joining us as a member of our DMD Class of 2025! Over the next few months, the Office of Admissions will use this webpage to provide you with important information pertaining to your matriculation in July. As new information is added to this page, we will alert you by sending you an email to the address we have on record through the Associated American Dental Schools Application Service (AADSAS).

Family Information Form
If you have not done so already, please complete the online Family Information Form. This information will be used to keep your family members informed of important and exciting information pertaining to Tufts University School of Dental Medicine:
Dean’s Certification of Disciplinary History Form
If you are currently enrolled in an undergraduate degree program, please remember that you must have a second Dean’s Certification of Disciplinary History Form completed upon graduation. Please provide this signed form to your undergraduate degree–granting institution and ask that they complete the form and send it directly to the Office of Admissions at the address listed at the top of the form. The form should be sent directly from the undergraduate degree–granting institution to the Office of Admissions and should arrive in a signed and sealed envelope no later than June 15, 2021.
