Forms and Costs for DMD and DIS Programs

Application Information and Forms

The Financial Aid Office provides all students with financial aid application packet on an annual basis. Packets are distributed shortly after January 1st each year and include complete instructions on applying for financial aid for the upcoming academic year. Paper documents are available to download in the menus below.

As a graduate/professional student, you are considered “independent” when applying for most federal student financial aid programs. However, some federal and institutional financial aid programs require the student to submit parental financial information as part of the student’s financial aid application requirements. Students are not required to submit parental financial information if they do not intend to apply for financial aid programs which require submission of such information.

Financial aid application procedures are outlined in the Financial Aid Manual which is included in financial aid application packets. All financial aid applicants must read the manual prior to the completion of any application forms. Applicants can refer to the Financial Aid Application Checklist for a summary of the required forms as well.

Financial Aid application documents may be scanned and uploaded utilizing the link below to our secure online portal or they may be mailed to our office. Electronic signatures will be temporarily accepted however typed signatures are not. Students who opt for the online portal should make sure their name and student ID are on each page, combine all forms into one or two files, and file name(s) should include student’s name, program and class year.

Students interested in applying for financial aid for the current 2022-2023 academic year, please contact the Financial Aid Office.

Cost of Attendance

A student’s cost of education is made up of both billed costs (tuition and fees) and indirect costs (living expenses, books, and supplies). Students may receive financial aid up to (but not exceeding) their total cost of education each academic year. Please note that the 2023-24 billed costs listed are subject to change.

The living expense allowance portion of a student’s cost of education is determined based on a survey of currently enrolled students and the US Department of Labor’s Bureau of Labor Statistics cost of living averages for the Boston metropolitan area. Currently, the living expense allowance is $3,150 per month for students living off-campus and $1,620 per month for those living with their parents.  This monthly allowance is multiplied by the length of the student’s academic year.  A more detailed breakdown of the components is available in the Student Loan Handbook. Information on housing options in the Boston area is available here.
